Responsibilities:
- Assist in development, implementation and revision of individual care plans.
- Assure that services provided are specified in the Care Plan.
- Monitor progress toward goals.
Required Skills:
- Teach, coach and educate the patient, family and/or caregiver about their disease process.
- Promote patient self management, problem solving and empowers patient.
- Assist patient with navigating the healthcare system.
Requirements:
- Two (2) years clinical experience as a RN required.
- Current CA Registered Nurse (RN) license.
- Strong interpersonal and written communication skills are essential.
