We’re looking for a Telephonic Triage Nurse to join our team, reporting to the Telephonic Triage Manger. This remote position involves assessing callers' symptoms, prioritizing calls, and advising patients/families on basic treatment or deciding when to send a nurse for evaluation. The shift is Sat-Sun 10am-6pm and Mon-Wed 5pm-1am EST.

We are looking for a full-time LPN passionate about all aspects of women’s health to provide direct patient care as part of an interdisciplinary care team and to serve as the first line of communication with patients in our clinical phone and messaging triage during office hours.

The command center vRN functions include collaborating with physicians, assessing patients status, interpreting data, and responding to patient problems appropriately. The nurse will virtually educate the patient and families on disease processes and procedures.

Alignment Health is seeking a remote nurse health specialist to join the virtual care center (VCC) team to provide triaging service for patients who call into the VCC. Expected to escalate patient calls to Advanced Practice Clinician (APC) when appropriate and is responsible for managing patient care and treatment in collaboration with the Physician and Nurse Practitioner/Physician Assistant.

Deliver coordinated, patient-centered virtual Care Management by telephone or messaging that improves members' health outcomes. You will specialize in guiding members through both routine and complex infertility and maternity clinical scenarios. Strong empathy and commitment to patient-centered care is needed.

The Planned Parenthood of Greater New York Centralized Follow-Up APC will spend most of their time providing virtual care to diagnose and treat issues related to reproductive, sexual health, abortion, and gender-affirming care, using EPIC, reviewing labs and diagnostic imaging, and handling prescription requests.
