The Registered Nurse, Care Line, is responsible for providing telephonic triage directional patient care advice for general and specific illnesses, health related issues, client counseling, patient advocacy, health education and referral and resource management to ChenMed patients and their families. Providing on-call coverage, provides remote clinical advice and Emergency Triage assessments within license and given technology.
Manages the after-hours on-call administrative operations for assigned branches and ensures that patient care and services are delivered appropriately and timely during after-hours and holiday coverage. Impacts patient, family and referral source satisfaction directly. The ideal candidate will be proficient with Homecare Homebase (HCHB), both PointCare and back office / R2 application.
Conduct video visits for chronic care management and remote patient monitoring to create an appropriate care plan for the member. Conduct care coordination and recommend/identify cost effective research based treatment and intervention. Utilize strong clinical skills in physical assessment and chronic disease management for at risk adults and apply member specific Care Management and individualized care planning.
