Job Description
Reema Health is seeking an experienced Licensed Mental Health Therapist to serve the behavioral healthcare needs of our members. This role involves providing care in an outpatient clinical setting, primarily via telehealth, but also in the home and community as needed. You'll be part of an Integrated Care Team, collaborating with Psychiatric Nurse Practitioners and non-clinical Community Guides to provide holistic care.
Primary responsibilities include assessing and diagnosing behavioral health conditions, formulating treatment plans, and ensuring a behavioral health perspective in comprehensive care planning. You will also complete initial intakes, deliver therapeutic interventions, and navigate members to community resources. Timely and appropriate documentation in our EMR.
Additional responsibilities include contributing to the evolving design and implementation of clinical pathways and protocols for therapeutic treatment of behavioral health conditions in Reema’s care model. Job responsibilities to grow and change as business necessitates.
