Provide behavioral health services and support social determinants of health as part of an interdisciplinary care team by conducting remote patient interactions via phone and video and performing assessments. Responsibilities include administering validated rating scales, developing behavioral health care plans based on patient progress, and providing psychosocial interventions tailored to individual needs.

Rendering evidence–based therapeutic treatment to patients. Providing an array of mental health services to promote individualized patient recovery primarily through tele-health video or telephone. Conducting intakes, individual, group, and family therapy when applicable. Providing mental health assessments, counseling, referrals, and intervention services.
