Patient Onboarding & Education:
- Conduct first session and early follow-up sessions to orient patients to the cardiac rehabilitation program including completing fitness testing
- Educate patients on program structure, expectations, and participation requirements, including group and individualized sessions
- Reinforce the importance of regular attendance, engagement, and completion of the program
Technical Support & Session Readiness:
- Assist patients with basic device and platform troubleshooting
- Guide patients step-by-step on how to join virtual sessions (audio, video, platform navigation)
- Ensure patients are technically prepared to independently access future sessions
Goal Setting & Behavioral Coaching:
- Support patients in establishing realistic, personalized health and recovery goals
- Provide motivation and emotional support, addressing common barriers such as anxiety, low confidence, or disengagement
- Utilize basic behavioral and coaching techniques to promote adherence and sustained participation
