Telehealth Physical Therapist (PT)

Patient Care:

• Manage referrals and schedule in a timely manner.
• Perform patient evaluations and virtual screenings, documenting all required information.
• Supplement virtual caseload with in-person house calls and SLC visits as needed.

Program Implementation & Coordination:

• Apply all aspects of virtual programs consistently, including tests, measures, and outcome tracking.
• Participate in virtual care marketing and telehealth training for field clinicians.
• Coordinate with regional clinicians for patient transitions and evaluation shadowing.

Performance & Compliance:

• Meet contract quota expectations on a bi-weekly basis.
• Maintain excellence in KPIs as monitored by leadership.
• Ensure compliance with FOX, federal, state, and payer regulations for telehealth services.

Professional Standards:

• Maintain professional attire and neat appearance at all times.
• Communicate effectively with team members and seniors.
• Obtain and maintain licensure in other states as determined by leadership.
