Responsibilities:
- Assist and coordinate with RN in member care delivery by communicating with providers and pharmacies.
- Address social determinants of health affecting members’ access to care.
- Provide education, emotional support, and connect members to resources.
- Track member interactions and support quality metrics.
Qualifications:
- LPN/LVN license in good standing within a compact state.
- Minimum 4 years’ LPN/LVN experience with oncology and infusions.
- Experience with care navigation and case management is strongly preferred.
- Bilingual in Spanish required.
