Behavioral Care Manager - W2 United States LCA – Lyra Pediatrics

Mental health coaching is part of Lyra’s preventative care offering and is a powerful way to manage emotions, challenge negative thinking, and reduce stress and anxiety. This role serves as the main point of communication and coordination between members, their internal care team, and external providers. The Behavioral Care Manager provides initial evaluations for new members and families, gathers pertinent information, screens for risk, and triages members to appropriate services. Work hours include Monday - Friday 11:30 AM - 8PM PST/ 2:30 PM to 11 PM EST. Primary point of contact for members and their care team providing initial evaluations, screening, and triage to appropriate services. Facilitate referrals for clinically indicated services outside the organization. Create accurate discharge summaries and plans to ensure members and families have the services and support needed for ongoing care.
