Medical Intake & Forms (De-Identified)

Task:

• Remove names, dates of birth, addresses, and identifiers before upload.
• Photograph or scan the entire form.
• Upload and confirm the de-identification checklist.

Requirements:

• iPhone running iOS 18.6 or later (Kled app is iPhone-only right now).
• Must be 18 or older.
• Use your phone camera inside the Kled app.

Compensation:

• $1.50 per accepted form, reviewed individually.
• Typical pace of about 3 minutes per form equals ~$30 per hour.
• Payouts via PayPal, Venmo, or Solana wallet in the app.

How to Start:

• Download Kled from the App Store and create an account.
• Open Tasks and search "Medical Intake & Forms (De-Identified)".
• Read the brief, follow steps, and submit. Accepted submissions are paid.
