Your Role:

  • Exceptional patient service and care coordination across disciplines.
  • Focus on identifying and addressing Social Determinants of Health.
  • Essential resource for patients navigating the healthcare system.

Primary Functions:

  • Perform telephonic outreach to enroll and engage patients.
  • Develop care plans with Nurse Care Manager addressing patient's needs.
  • Facilitate seamless transitions of care across settings.

Required Qualifications:

  • 3 years of experience in health care coordination.
  • Excellent knowledge of community resources.
  • Strong communication and teaching skills.
