As a Post Acute Nurse Care Manager, you will collaborate with Skilled Nursing Facilities to manage nephrology patients receiving skilled nursing care after an acute hospitalization, focusing on ensuring their skilled nursing stay promotes efficiency and meets their specialized needs with a coordinated plan to discharge to home or community.

Manage a team dedicated to providing longitudinal care management for patients with kidney disease as a Program Manager. Serve as a collaborative and supportive leader responsible for the day-to-day operations of your care management team, focusing on proactive, person-centered care. Primary duties include managing the performance of team members, providing ongoing training and development, and ensuring adherence to best practices and protocols.

As a Manager, Strategic Finance with Evergreen Nephrology, you are responsible for providing financial reporting and analysis for Evergreen’s corporate teams and physician partners. This includes developing financial models, preparing variance reports and creating presentations to be used in internal and external meetings with key stakeholders. You will collaborate with teams across the organization, including our accounting, analytics, actuary, and operations teams.
