Job Description

The Nurse Navigator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. Responsibilities include coordinating multidisciplinary care to the patient using the nursing process. They collaborate with the patient, family, physicians, and staff while maintaining ANA standards. The nurse assesses and reassesses the patient and family for ongoing care needs and initiates referrals to appropriate resources. This role involves data collection, analysis, and coordination of care. They triage information, schedule patients, assist in creating care plans, and collaborate with the healthcare team. Furthermore, they utilize research findings to guide practice decisions, stay aware of current research trials, and serve as a resource to non-clinical staff. They perform patient care, optimize safety, reduce errors, identify learning needs, educate patients and families, and assist with transitions in care and providers.
