Clinical Intake Specialist

In this remote role, you will perform care management duties through HIPAA-compliant hardware and software. You'll communicate critical information and client preferences to the care team, conduct outreach to newly referred clients, and assist clients in completing initial intake visits, psychosocial assessments, and care plans. Responsibilities also include reviewing medical charts, documenting client medical needs, utilizing evidence-based practices like Motivational Interviewing and Trauma-Informed Care, and attending weekly Team Operations Meetings. The successful candidate will conduct 8-12 intake visits daily, maintain an organized schedule, perform proactive outreach, and stay available for warm transfers from field teams, thriving in a fast-paced environment while meeting key performance metrics.
