Licensed Clinical Social Worker (LCSW)

Provide clinical care as part of a multidisciplinary team offering remote, virtual-first care to seniors with serious mental illness (SMI) and substance use disorders (SUD). This LCSW will be instrumental in delivering compassionate and comprehensive care to a vulnerable population, leveraging their expertise in mental health to improve patient outcomes. The role requires a deep understanding of the challenges faced by individuals with SMI and the ability to provide tailored interventions through a telehealth platform. Conduct virtual initial assessments and ongoing care for seniors with serious mental illness (SMI) and substance use disorders (SUD), in collaboration with psychiatrists and other healthcare professionals. Develop comprehensive treatment plans, which may include coordinating psychotherapy or other treatment modalities, and assessing the need for referrals to specialized care. Collaborate effectively as part of a multidisciplinary team to address the holistic needs of patients, including their physical health, behavioral health, and social well-being. Provide patient care and education tailored to the individual’s age, abilities, educational background, and cultural context, often involving family members in the care process. Coordinate care with primary care physicians (PCPs) and other medical professionals to ensure a cohesive approach to patient health. Offer timely psychiatric urgent care and be responsive to patients’ immediate needs. Document all clinical work comprehensively and promptly in the electronic health record (EHR). Adapt to a novel care delivery paradigm and thrive in a fast-paced environment.
