Responsibilities:
- Develop care plans and assess patient health status through outbound calls to identify gaps in treatment.
- Coordinate care and make referrals to outside sources, ensuring necessary services post-discharge.
Qualifications:
- Current nursing license in Louisiana with 2 years of professional nursing or care management experience.
- Acute hospital experience is needed, and EPIC & InterQual experience is preferred.
