Perform comprehensive triage of referrals and medical records to determine urgency, risk, and appropriate care pathway.
Deliver telephone/video triage using internal policies and best practice to support decision making.

Care Coordination:

Provide accurate and timeous record keeping
Signpost internally and externally, escalate and refer internally as required.

Service Development & Quality:

Lead or contribute to pathway audits, SOP updates, and outcome measurement.
Support creation of patient education materials and digital triage scripts.
Champion clinical governance, incident reporting, and continuous improvement.
