Job Description

The Admission Coordinator is responsible for regional coordination of patient referrals, acting as a resource for referral sources. They work closely with branch staff to ensure adequate coordination and transition to hospice services. Occasional weekends and holidays are required. This is a fully remote position; however, candidates must reside in a state where the company operates. Responsibilities include receiving and documenting referrals, gathering required documentation for admission, ensuring clear order documentation, obtaining agreements for admission from patients meeting criteria, and communicating referral status with relevant staff. The role also involves managing calls, following up with patients, and coordinating referrals with sales staff.
