Sr. Director of Clinical Practice

Identify, create, and implement home health clinical practice standards for in-home care management that deliver high value clinical and financial outcomes in support of our home health offering to patients, physicians, health systems, and payer partnerships. Primary Focus: Establish in-home clinical assessment and practice standards that align with in-home procedures and policies Establish protocols and standards for the in-home disciplinary care team to improve patient experience and mitigation of readmission. Use evidence, train, and implement care standards in each HH region that drives favorable clinical and financial outcomes.
