Social Worker Care Manager

Ennoble Care is seeking an experienced Full-time Behavioral Health Specialist for our department supporting patients within and around the Hoboken / Jersey City, NJ area. This position is primarily remote, with the possibility of occasional in-person visits in the future based on patient needs. Responsibilities include supporting the mental and physical health care of patients, screening and assessing for mental health and substance abuse disorders, facilitating patient engagement, and providing patient education. This role involves tracking treatment response, monitoring patients for changes in clinical symptoms, and supporting psychotropic medication management. The care manager will provide brief behavioral interventions, facilitate referrals for psychosocial treatments, and participate in regular caseload consultations with psychiatric consultants. They will track patient follow-up, document encounters, and facilitate treatment plan changes as needed. Additionally, the role requires facilitating referrals for external services such as housing assistance and vocational rehabilitation, and developing relapse prevention plans with patients. Comfort with using Excel and Word, along with strong time-management skills, is essential in effectively managing daily tasks through calendars and the EMR system.
