Responsibilities:

  • Proactively identify patients suitable for hospice care and track them to ensure continuity during transition.
  • Send referrals as directed and collaborate closely with the Director of Care Transitions on special projects.

Qualifications:

  • Must be an LPN or RN with at least 3 years of clinical home care and/or hospice experience.
  • Requires strong knowledge of post-acute care services and state/federal regulations and compliance.
  • Excellent analytical, problem-solving, and interpersonal skills with fluency in English are essential.
