Responsibilities:
- Review referral information to identify priority, risk, and required OT intervention.
- Carry out remote assessments via telephone and video call.
- Provide preventative advice, guidance, and problem‑solving support to adults, carers, and professionals.
Qualifications & Experience:
- Experience working in Adult Social Care, community OT, hospital discharge, reablement, or similar settings.
- Experience managing complex cases and risk in fast‑paced environments.
- Experience completing remote assessments.
Knowledge & Skills:
- Strong understanding of: Care Act 2014 Housing legislation Equipment provision & adaptation pathways Strength‑based approaches
- Excellent digital and telephone communication skills.
- Ability to assess risk, problem‑solve creatively, and make safe, defensible decisions.
