Intake Assessor - LPN Remote in Iowa

The Intake Assessor role involves several key responsibilities:

• Conducting front line intake clinical screening to members.
• Assessing urgency of assessment referrals and determining members acuity level.
• Determining the type of assessment most applicable for members based on their health care needs.

Additional duties and responsibilities:

• Assisting with scheduling and conducting express assessments as needed.
• Performing virtual assessments of persons with disabilities.
• Validating the appropriate treatment plan.

Key requirements for the role:

• A two or four-year nursing degree and/or equivalent training and/or experience
• 1-2 years of clinical experience required
• Current and valid LPN/RN licensure in Colorado
