Job Description
Communication:
- Excellent oral and written communication skills
- Positive interpersonal and telephone skills required
- Communicating care needs to the interdisciplinary team
Responsibilities:
- Request and obtain patient records
- Maintain patient health records
- Assist with TARS/Authorizations and communication with pharmacies
Coordination:
- Schedule HV/VC visits for the clinical team
- Coordinate transportation for patient medical appointments
- Monitor and request the ordering of office supplies
