Special Programs Care Coordinator (Remote)

This dual-role coordinator will play a critical part in driving enrollment, educating patients and families, and supporting care teams through high-impact, multitasking responsibilities across both programs. The ideal candidate thrives in a fast-paced, evolving environment and is eager to grow alongside innovative healthcare initiatives. Responsibilities include conducting eligibility screenings for the BHI and GUIDE programs, performing outreach to patients and caregivers, and educating them on the purpose and benefits of the programs. The coordinator will also assist with marketing, serve as the first point of contact for questions, collaborate with the interdisciplinary team, and help coordinate resources and appointments.
