Care Manager - CBHC Lynn

Eliot is seeking motivated Care Managers to join our Community Behavioral Health Center (CBHC) which serves as a single entry point for mental health and substance use services for youth, families and adults. The ideal candidate will provide in-home support and services to clients, primarily focusing care coordination and ensuring that treatment goals are culturally competent, realistic, attainable and promote recovery. Responsibilities include providing client support focusing on problem-solving, skills training, behavior modeling, optimism, and encouragement, delivering interventions in various settings, enrolling eligible individuals in care management programs, assisting with ongoing assessment of symptoms, and contributing to the development of treatment plans. Each individual should be actively involved in their treatment planning process and timely face-to-face contact with enrollees in the community following inpatient discharge should be ensured.
