Responsibilities:
- Oversee patient care activities and manage visit requests based on clinical priorities across assigned markets.
- Facilitate IDG meetings, review patient conditions, and ensure accurate care planning and timely documentation.
- Support adherence to Medicare, Medicaid, ACHC, and JCAHO regulations and promote a culture of continuous improvement.
Qualifications:
- Current RN license and at least 3 years of direct patient care experience.
- Minimum 2 years of experience in a hospice or home-based care setting with strong regulatory knowledge.
- Excellent communication, problem-solving, and leadership skills to support diverse clinical teams.
