Job Description

Coordinate care, support teams, and improve access after hours as a Remote Afterhours Coordinator. This telephonic, work-from-home position reports to the Manager of Afterhours Clinical Support and plays a vital role in supporting After Hours workflow, scheduling visits, referral coordination, and assisting on-call nurses (RNs). Main responsibilities include coordinating afterhours patient care workflows and supporting on-call RNs. You will also schedule patient visits using HCHB (Homecare Homebase) system, review medical records using nationally recognized clinical criteria guidelines, and process referrals.
