About the Role:
- Remote RN Care Coordinator supporting safe transitions from hospital to home.
- Provide telephonic care coordination and education to high-risk patients.
What You'll Do:
- Conduct post-discharge outreach and assessments in Epic work queues.
- Coordinate services, medication reconciliation, and community resources.
- Collaborate with providers and adapt to weekend/holiday rotation.
Qualifications:
- Active RN license and at least 1 year of clinical experience.
- Home care or ambulatory care experience preferred; EPIC familiarity a plus.
