Chronic Care Manager

We are seeking a compassionate and patient-centered Licensed Practical Nurse (LPN) to join our care team as a Chronic Care Manager. This fully remote position plays a key role in supporting patients with chronic conditions by providing proactive, ongoing care coordination and education. Ideal candidates are highly organized, strong communicators, and committed to making a difference in patients’ day-to-day lives. Key Responsibilities: Conduct comprehensive patient assessments to develop individualized care plans. Collaborate with interdisciplinary care teams, including hospitals and primary care providers. Monitor patient health status and facilitate follow-up care. Educate patients and families about self-management, medications, and available resources. Maintain accurate and timely documentation in compliance with regulatory standards. Advocate for patient needs and coordinate services to ensure continuity of care.
