Overview:
- Primary logistical architect for patients and family caregivers.
- Navigate the Medicare system and ensure seamless care.
- Act as a high-level project manager for the patient’s health journey.
Responsibilities:
- Provide guidance on Medicare, benefits, and enrollment logistics.
- Review Medicare Summary Notices for errors and file appeals.
- Schedule specialist visits and manage transportation logistics.
Requirements:
- Minimum of two years' experience in a non-clinical social worker environment.
- Active Social Work license (LSW, LMSW, or equivalent).
- Deep knowledge of Medicare regulations and prescription coverage.
