Hospice Triage Nurse

Schedule and Location:

• Work schedule is 5 days per week, Tuesday through Saturday, from 8:30 AM to 5:00 PM.
• This is a fully remote position, requiring a California RN license.

Essential Duties:

• Receive and triage phone calls from hospice patients and families to assess their immediate needs.
• Coordinate necessary care over the phone and schedule in-person home visits with appropriate clinicians.
• Manage patient pain and symptoms, obtaining updated physician orders and modifying care plans accordingly.

Qualifications and Requirements:

• Minimum of three years of recent triage/telephonic nurse experience or one year of RN experience in hospice/palliative care.
• Must possess a current California RN license; CHPN certification is preferred for this role.
• Must comply with all CDPH vaccine requirements for healthcare personnel.
