Patient Education and Care Coordination:
- Educate patients and families on CKD management and modality choices.
- Coordinate services with interdisciplinary team (social worker, dietitian, pharmacist).
- Review medical records and update REACH EMR.
Qualifications:
- Graduation from an accredited nursing program (BSN, AA, diploma).
- Current Registered Nurse license.
- Must be 21+ with valid driver’s license and acceptable driving history.
