Job Description
The Project Manager for Community Health and Housing is supporting the design, implementation and evaluation of programs across CommonSpirit Health that address housing and homelessness. The position is focused on managing the data and evaluation to support system and local efforts to better serve our populations experiencing housing insecurities and homelessness. This position will also support the expansion of the Research, Equity and Advisory Council on Housing Insecurities (REACHI).
The Project Manager for Community Health and Housing interacts with community partners and supports housing and homelessness related projects nationally to establish shared metrics and identify opportunities to scale best practices. The manager works across multidisciplinary teams including care coordination, nursing, mission, and service area/facility based community health leadership. This position supports work to set priorities, develop strategies and initiatives, and build community based partnerships to improve homeless services capacity including housing, supportive services such as transitions of care, emergency resources and areas of prevention.
