The Registered Dietitian delivers nutrition therapy to clients in intensive programming. He/she performs nutrition assessments, creates personalized care plans, monitors ongoing medical and nutritional conditions, and tracks client progress. He/she provides nutrition education to enhance clients' nutritional health and their relationship with food. This position requires flexibility to float to various locations and departments based on operational needs.
Conduct comprehensive assessments, including functional behavior assessments (FBAs) and skill assessments; develop and monitor individualized ABA treatment plans for clients, setting measurable goals and objectives; supervise, train, and support RBTs in the implementation of treatment plans; and evaluate client progress and adjust treatment plans as necessary.
This part-time Speech Pathologist role (22.8 hours per week) involves providing assessment and intervention to clients with physical, cognitive, behavioural and emotional issues, primarily from acquired brain injuries. The case-mix is diverse, requiring close collaboration with other disciplines. You will have the ability to work remotely, with flexible start and finish locations.On-site attendance is required once per week/fortnight.
Provides telephonic and in-home assessments of members' medical, psychosocial, physical and spiritual needs. Develops, implements and monitors care plan, ensures continuity of care and monitors member's chronic care benefit.
