Performs clinical triage review of inbound member and patient calls to ensure that services rendered to customers meet clinically appropriate direction and triage, utilizing clinical skills to coordinate, document, and communicate all aspects of the appropriate clinical advice and triage. The role involves consulting with Medical Directors, referring callers to venues of care, and may work varied shifts.

The Care Coordinator ensures linkages for CCBHC patients and the community across the health services spectrum, including physical and behavioral health. They work collaboratively with staff, providers, and community resources to assist clients in overcoming barriers to care and improving health outcomes.
