Provide support to members facing substance-exposed living and those in recovery, coordinating care with the Substance Use Disorder Program team.
Complete documentation for member contacts, assessments, and care plans, and communicate professionally with members, families, and community agencies.
Participate in case management meetings, achieve set goals and Key Performance Indicators, and represent Mindoula in community outreach efforts.
