Role Overview:
- Serve as role models for individuals facing substance-exposed living.
- Plan care, assess client needs, and complete screenings.
- Refer clients to appropriate resources.
Responsibilities:
- Identifying individual member needs and documenting interventions.
- Providing support to members through a variety of settings.
- Contributing to case management meetings to ensure a team approach.
