Responsibilities:
- Work under the direction of the Manager, Behavioral Health.
- Promote patient autonomy.
- Address barriers to support treatment recommendations.
Qualifications:
- Completion of 4 years of high school plus additional training.
- Bachelors degree preferred.
- Must have basic competency with documentation and computer skills.
Experience:
- Community Health Worker and/or Recovery Coach certification preferred.
- Experience with EMR in medical settings preferred.
- Bilingual skills preferred.
