Job Description
Support Hazel’s Physical Health provider team with phone calls to families in both English and Spanish. Manage internal chat communications among providers, relay messages to appropriate staff, and create job tickets for school/initiator needs. Provide interpretation and translation services for phone calls, telehealth visits and written medical documentation. Answers parent/guardian questions and relay pertinent information back to the provider. Troubleshoot minor technical difficulties and escalate to Hazel’s IT team, when needed. Work with the Clinical Operations team to identify process improvements and document new procedures. Ensure continuity of care by acting as a liaison between Hazel providers, parents/guardians, students and school personnel. Provide back-up coverage to other support staff.
