Job Description
Key Responsibilities:
- Provide direct patient care for Nourish patients, many of whom have metabolic conditions
- Conduct telehealth visits focused on evaluation, treatment planning, and ongoing management
- Collaborate with RDs and physicians to design comprehensive care plans integrating medical therapy and lifestyle change
Qualifications & Requirements:
- Active and unrestricted Nurse Practitioner license
- Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), or equivalent
- Minimum of 2 years of experience managing patients with metabolic conditions
