Outreach Specialist - Scheduling

The Scheduling Outreach Specialist is responsible for conducting emergency department, wellness, or preventative service outreach calls to identified patients. This role will provide patient education about clinic appointment availability and identify and communicate if the patient has any care coordination or triaging needs from the supported practice. Essential duties include conducting direct telephonic outreach, applying motivational interviewing tactics, acting as a liaison for patients and practices, responding to inquiries, educating patients on services, and tracking outreach outcomes. The specialist will identify outreach trends, maintain collaborative relationships, and perform other assigned duties. Required skills include understanding call center operations, experience in a medical setting, proficiency with technology, strong communication and motivational skills, and access to reliable high-speed internet.
