Job Description
The Continuing Care Patient Navigator's main responsibility is to assist patients, members of the department, and the healthcare team while also collaborating with colleagues across the care coordination continuum. Responsibilities include working together with team members to reach member goals, providing telephone assistance to members, and taking care of other clerical tasks. Candidates must have strong communication and interpersonal skills in order to effectively communicate with patients and healthcare professionals. Candidates must also have experience with electronic medical records (EMR) and be proficient in basic computer skills. This role requires the candidate to work from 600-1430 Monday through Friday. While the role is primarily remote, it does require some in-person meetings at our corporate office regularly.
