Nurse Practitioner - National After-Hours Team

Scheduling & Hours:

• Work 24 hours per week on a set schedule, providing coverage on weeknights from 5pm to 8am local time and continuous coverage from Friday 5pm to Monday 8am.
• Cover every other weekend with 12-hour shifts on both Saturday and Sunday, and fulfill overnight and holiday coverage requirements including two 12-hour holiday shifts per year.

Qualifications:

• Must hold a Master's Degree as a Nurse Practitioner with current unrestricted NP licenses in Iowa, Missouri, Kansas, or Ohio, and be able to obtain multi-state licenses.
• Requires national certification as ANP, FNP, or GNP, a background in acute and chronic disease management, and 3+ years of NP experience.
• Must be proficient with computer skills and electronic medical records, and able to work scheduled shifts independently.

Role Impact:

• Provide compassionate telephonic care and clinical direction during evenings, nights, weekends, and holidays to ensure residents receive timely support.
• Manage high call volumes and perform telehealth visits, evaluating acute changes, falls, and controlled substance visits to prevent unnecessary transfers.
• Act as a trusted clinical partner, making an immediate impact in the lives of older adults by supporting safe, effective treatment in place.
