Palliative Care Licensed Clinical Social Worker (EST)

Responsibilities:

• Conduct comprehensive assessments of psychosocial distress, mental health needs, caregiver burden, and social determinants of health.
• Facilitate serious illness conversations and advance care planning, documenting goals of care.
• Provide clinical recommendations and escalation to physicians and advanced practice providers in accordance with protocols.

Qualifications:

• Master's in Social Work from an accredited program and active LCSW license.
• Clinical experience in palliative care, including advance care planning and serious illness conversations.
• Demonstrated ability to exercise independent clinical judgment and collaborate in an interdisciplinary team.
