Job Description
The National After-Hours Call Team delivers telephonic care and direction to residents and facility staff during overnight, weekend, and holiday hours. The role involves delivering medical care to patients across multiple states to ensure high quality continuity of care and treatment. Working from home, you'll provide after-hours virtual care for aging residents in various settings.
Responsibilities include coordinating care for residents between facilities, hospitals, and clinics using Curanaβs telephonic platform, utilizing EMR to provide acute care, delivering cost-effective care, managing chronic and acute conditions in collaboration with physicians, performing comprehensive assessments, and ensuring encounter documentation supports diagnoses. Part-time (24 hours/week) with weeknights, weekend day shifts, and holiday coverage required.
