Job Summary:
- Assesses, plans, and coordinates comprehensive care plans to optimize patient outcomes and resource utilization.

Requirements:
- Bachelor's degree in Nursing preferred; current RN license in state(s) of practice required.
- Minimum four years clinical experience in case management, home health, critical care, or related areas.
- Case management certification required within three years of hire (CCM, ACM, or RN-BC).
Responsibilities:
- Serves as a liaison among patients, families, physicians, and providers to ensure coordinated care.
- Provides specialized education on disease conditions and stays current with medical trends.
- Works proactively with referrals to identify candidates for case management and maintains patient caseload.
