About the Opportunity:
- As a Case Management Case Manager, you will conduct comprehensive assessments and develop individualized care plans.
- You will coordinate access to services, collaborate with providers, and monitor client progress.
- Your role involves educating clients and families while maintaining accurate documentation.
Qualifications:
- Bachelor’s degree in nursing, social work, or psychology required.
- Certified Case Manager (CCM) must be obtained within 2 years.
- Prior experience in care/case & disease management or related healthcare role.
Your Key Responsibilities:
- Assess client needs and create tailored care plans.
- Coordinate with healthcare providers and insurance companies for seamless care.
- Participate in case conferences and stay updated on industry trends.
