About the Role:
- Support care continuity and patient safety by identifying and closing care gaps.
- Work closely with the clinical team to ensure information integrity within the EHR.
- Coordinate transitions of care, medication safety, and timely access to specialty services.
What You'll Do:
- Complete biopsychosocial assessments and proactively resolve gaps in patient records.
- Schedule post-discharge follow-ups and track labs, imaging, and screenings.
- Facilitate behavioral health follow-through and coordinate DME and community resources.
What We're Looking For:
- Licensed practical nurse, registered nurse, or social worker with case management experience.
- Proficiency with athenahealth and familiarity with health information exchanges.
- Strong organizational skills and ability to manage multi-patient registries.
