Responsibilities:
- Manage patient safety alerts and scrub charts for health maintenance needs.
- Perform daily outreach to schedule appointments and coordinate services.

Qualifications:
- Two years in a hospital or clinic setting with high school diploma or equivalent.
- Must have medical assistant training, EMT license, or LPN license.
- Proficiency in EPIC, Microsoft Office, and strong analytic skills.
